What is an Acknowledgment of Paternity?
✅ The Acknowledgment of Paternity is the process for requesting that the father’s information be added to the child’s birth certificate in the Puerto Rico Demographic Registry.
✅ The process may also involve updating the child's last names, when applicable, in accordance with the requirements of the Demographic Registry.
Preliminary evaluation required
Important: This process must be evaluated before placing an order, as it depends on the current information on the certificate, the availability of both parents to sign, and any additional legal or registration issues that may arise.

Main requirement: Both parents must sign
⚠️ For this procedure, both parents must agree and be available to sign the required documents under oath before a notary.
✅ If either parent is outside of Puerto Rico, they can usually sign the document before a notary public where they are located, following the specific instructions we will provide.
Additional errors in the certificate
⚠️ This service is intended for Acknowledgment of Paternity—that is, adding the father’s information to the birth certificate and, when applicable, updating the child’s last names as part of that process.
✅ If the certificate contains other errors or additional issues, please include them on the same Acknowledgment of Paternity Evaluation Form so that we can review the entire case.
